A grieving mother, Kath Brace, sadly ended her life seven months after the heartbreaking loss of her newborn baby, Otis, due to complications. Despite being admitted to a mental health facility, Kath was granted unsupervised leave and was found deceased in nearby woodland. An inquest jury attributed contributing factors to her death to shortcomings in her care.
